3. Register your biometric information (front-facing photograph, fingerprints, and iris scan)
4. Wait for PhilSys Number (PSN) to be generated

Philippine National ID Requirements
To get your national ID, you’ll need to submit the following requirements:
- PhilSys Application Form (Form No.1) – Applicants must fill out and submit the application form to any registration center (see below).
- Primary Documents – Aside from the PhilSys application form, any of the following documents should also be submitted:
- PSA birth certificate and one (1) government-issued ID showing your full name, photo, signature, and thumb mark
- Valid Philippine passport
- Unified Multi-purpose Identification (UMID) card from Government Service Insurance System (GSIS) or Social Security System (SSS)
- Other documents approved by the PSA
- Secondary Documents (optional) – You can submit any of these documents if you don’t have any primary documents:
- PSA birth certificate
- Government-issued ID showing your full name, photo, signature, and thumb mark
- Equivalent ID issued by the private sector approved by the Philippine Statistics Authority (PSA)
For resident aliens, they must present their Alien Certificate of Registration (ACR) or ACR ID.
For married women that have their husband’s surname, they must submit a PSA marriage certificate.
